POSITION TITLE:        Pediatric Nurse Practitioner (PNP)
RESPONSIBLE TO:     Practice Manager, Medical Director
FLSA STATUS:            Full time – Exempt, Part time – Non-Exempt

Purpose of this Position:  The Pediatric Nurse Practitioner (PNP) is an interdependent member of a health care team and will function within established state and federal rules and nursing standards of care. The incumbent will be required to exercise sound professional judgment while providing services to ambulatory patients, up to the age of 21, within the specific population served by the clinic. Patient assessment, diagnosis and therapeutic measures will be carried out in close association with the clinic physicians, appropriate specialists and other care agencies in the community. The incumbent will participate in staff meetings, conferences, in-services, training, continuing education and long and short-term planning for Valley Family Health Care.

QUALIFICATIONS:

1. Current Pediatric Nurse Practitioner Licensure in Oregon and Idaho.
2. Successful completion of advanced practitioner training with Master of Science Nursing or Doctorate of Nursing Practice degree with specialty in Pediatrics from an approved Pediatric Nurse Practitioner program is required.  Minimum three years of professional nursing experience; at least 2-3 years’ nurse practitioner experience in pediatric setting, preferred.
3. Must maintain active Drug Enforcement Administration licensure.
4. Current Basic Life Support (BLS) certification
5. Pediatric Advanced Life Support (PALS) to be obtained within 90 days of hire.
6. Preferably Bilingual - English/Spanish

RESPONSIBILITIES:

1. Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing/presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.
2. Order/perform appropriate laboratory diagnostic and other screening tests. Seek other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. 
3. Integrate data to determine diagnosis and therapeutic plan, including identification of any health risks.
4. Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Handle minor lacerations. 
5. Communicate appropriate case management information to other professionals and community agencies. 
6. Prepare documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.
7. Participate in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. 
8. Act as consultant/collaborator to clinical team on area of specialty. Comply with patient confidentiality requirements. Promote patient advocacy.
9. Collaborate with physicians in managing acute and long-term medical needs of patient.
10. Assist with meeting quality measures by completing tasks related to VFHC initiatives.
11. Perform other duties as assigned.

Patient Centered Care:

1. Instruct patient/family regarding medications and treatments. Educate patients regarding health promotion/illness prevention. Recommend appropriate community resources to meet patient/family needs. 
2. Support patient self-management of disease and behavior modification interventions.
3. Coordinate continuity of patient care with external healthcare organizations and facilities, including referrals to specialty care providers, and continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
4. Promote clear communication among the care team and treating clinicians by ensuring awareness regarding patient care plans.
5. Provide monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.                                                                                                                                           

Quality Improvement:

Participate in improving VFHC performance, processes, or programs through quality improvement which will be demonstrated in at least one instance annually.

Physical Requirements:

1. Able to lift up to 25 lbs.
2. Continuous standing, walking, stooping, bending, kneeling.
3. Normal manual dexterity.
4. Correctable vision and hearing.
5. Must be able to read, write and speak clearly.
6. Must have proficient keyboarding skills.
