• Virtual care focus on Diabetes and Hypertension best practices
• How to effectively deliver care for patients with diabetes and/or hypertension through virtual care workflows and the use of the whole care team

We've designed this session with Health Management Associates (HMA) to target not only Care Managers/Coordinators, but also Leadership Teams, including CFOs, COOs and CMOs, and Care Management Supervisors/Leads.
