Patient Engagement and Medication Management for Diabetes and Hypertension Webinar Series

The Patient Engagement and Medication Management for Diabetes and Hypertension webinar training series is focused on engaging patients and medication compliance alongside pharmaceutical programs and their staff. This series will focus on multiple aspects of the pharmacy in patient care including organization of pharmacy in primary care, medication reconciliation and compliance, patient and staff engagement, as well as pharmacy integration into patient care.

The series will be led by Dr. Holmes a clinical pharmacist and Assistant Professor of Pharmacy Practice and Family Medicine at Idaho State University (ISU).

The Types and Importance of Pharmacy Services in Primary Care looks at the role that pharmacy plays in primary care around:

 Medication Related Problems in Primary Care

 Why Having a Medication Expert is Important

 Medication Related Problems in Hypertension and Diabetes

 Types of pharmacy services that are available- roles and services they provide

    • Annual Visits being done by pharmacists
    • Logistics of having a pharmacy and pharmacist in place
