The ICHCA Care Management Workgroup brings together Community Health Workers, RN Care Managers, Care Coordinators, Patient Navigators, and Value-Based Care leadership from across Idaho's Community Health Centers.
Led by Susan Ault, the group meets bimonthly on the first Wednesday of every other month.
