Diabetes, Heart Disease & Stroke Prevention Program
The Idaho Diabetes, Heart Disease, and Stroke Prevention (DHDSP) Program is currently seeking applications from Idaho organizations to continue implementation of the Rx for Fresh Fruits and Vegetables (Rx for FFV) Program and to automate the provision of nutrition vouchers for priority populations. The priority populations to be served under this funding opportunity includes American Indian/Alaskan Natives, persons of Hispanic/Latino ethnicity, and/or persons of low socioeconomic status living in Idaho.
The Idaho DHDSP Program is housed in the Bureau of Community Health (BCH), Division of Public Health. These opportunities address the priorities of the cooperative agreement funded by the Centers for Disease Control and Prevention (CDC) “A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes” (CDC-RFA-DP-23-0020, CFDA# 93.988).

Improving Access to Healthy Foods and Participation in Evidence-based Diabetes Prevention and Management Programs
The primary goal of this funding opportunity is to automate the provision of nutrition vouchers for the Rx for FFV Program to the most efficient, electronic technology feasible while concurrently enrolling priority populations with a diagnosis of prediabetes or diabetes. Funding per subgrantee organization under this announcement will be up to $52,200, with an estimated start date of November 1, 2023, and ending on June 29, 2024. The Idaho DHDSP Program plans to fund at least one organization.

A webinar will be hosted by the Idaho DHDSP Program on Tuesday, September 12, 2023, from 11:30 a.m. – 12:30 p.m. MDT for all eligible organizations to ask questions related to this funding opportunity.
