Session Description:
As Social Determinants of Health and Value-Based Care reporting continues to grow, the capture of SDOH and Diabetes Z codes is vital. This session will explore ways to capture Z codes using documentation gathered throughout the encounter to increase Z code reporting.
The session will open with an overview of Z codes and their importance to the entire clinic team and move to where Z code documentation can be extracted from, - it starts with initial patient contact and ends with provider narrative. Finally, ways synergistic clinic teams can collaborate using current documentation to capture Z codes.
Learning Objectives:
- Define Z-codes and why they matter to each member of the team, from front desk to billing.
- Review specific documentation rules/requirements for SDOH and DM Z-code reporting.
- Discover ways to utilize current documentation, beyond provider notes, to capture SDOH and DM Z codes.
The presenters will answer audience questions about VCC's quality improvement efforts and where automation can improve health center performance.
