Partner Access Data Portal (PDAP) Resources

Access to this system will require the following

  1. That each health center sign a memorandum of understanding with DHW.
  2. Each enrollment counselor sign a user agreement.
  3. ECs will obtain releases of information from each client prior to accessing personal information in PDAP. DHW also asks that you keep records of signed user agreements and releases of information on file for DHW audit purposes AND allow DHW access to these items for annual audit purposes. 
  4. Email the completed and signed User Agreement to PartnerAccess@dhw.idaho.gov.

You do not have to create a release for consumers to sign. DHW will accept the form you are currently using for each person who you assist with enrollment, the consumer authorization form

Please Note: DHW requires health centers to inform them within 5 days if an EC leaves the position or if the person no longer needs to use PDAP for their work. The account must be deactivated for privacy reasons.
